Philippine Health Insurance Corporation (PhilHealth)
PhilHealth Registration
First-time registrants accomplish the PhilHealth Member Registration Form (PMRF) and submit proof of identity (birth certificate, baptismal certificate, or valid government ID). Newly hired employees without a PIN usually submit two PMRF copies to HR and wait for the MDR/ID through the employer. Direct or self-paying members file the PMRF and supporting documents at a Local Health Insurance Office, PhilHealth Express, or an accredited registration partner.
How to register as a PhilHealth member and obtain a PhilHealth Identification Number (PIN) using the PMRF—covering employed and direct/self-paying routes without medical or benefit advice.

PMRF, identity proof, ER-2/employer path, and no enrollment fee come from PhilHealth’s formal-member page, PMRF reminders, and Citizen’s Charter. Premium rates and benefit eligibility are out of scope.
