How To Do It
- 1
Prepare an affidavit if the card is lost or unreadable
Confirm at officeOlder LTO charter text asks for an Affidavit of Loss or Mutilated ID. Ask the licensing office whether a police blotter is also required in your case; this site does not treat a blotter as a confirmed current nationwide requirement.
- 2
File a miscellaneous license transaction
Official sourceCurrent LTO service coverage includes duplicate driver’s license, student permit, revision of records, and storage/reactivation. Tell the Client Service Officer you need a duplicate card.
- 3
Pay the published duplicate-license fees
Official sourceLTO’s license fee schedule publishes a duplicate valid driver’s license total of ₱355, or ₱455 with revision of records. The 2025 miscellaneous-service page also shows ₱325 for a revision-of-records variant. Pay only the amount the cashier assesses for your case.
- 4

Important Notes
Confirm the lost-card checklist at the window
Confirm at officeThe 2025 Citizen’s Charter page for this service is combined with revision of records, storage, and reactivation, and the extracted checklist is dominated by revision-of-records documents. Items marked “confirm at the office” come from the older dedicated duplicate-license list.
A duplicate is not a renewal
Confirm at officeIf the license is also due for renewal or already expired, you may be assessed renewal fees and requirements instead of, or in addition to, duplicate-card fees.
Storage is a different service
Official sourceLTO also publishes storage of a valid license. That is not the same as replacing a lost card.
Where / How To Apply
LTO License Section and other licensing offices that process miscellaneous license transactions, generally 8:00 AM to 5:00 PM.
Confirm whether your office treats a plain duplicate as that same service clock, especially if revision of records is also requested.
