PRC Duplicate / Replacement Professional ID Card

For a lost or damaged PRC Professional Identification Card, use LERIS Duplicate (not renewal). Log in at online.prc.gov.ph, select Duplicate, secure an appointment, pay, and present the system-generated Action Sheet plus an Affidavit of Loss or damaged PIC. PRC FAQ states the application fee is ₱250 and duplicate PIC is generally released within the day when requirements are complete.
