Important Notes
Keep photocopies of OR and CR once you have them
Confirm at officeReplacing lost papers takes records confirmation time. Photocopies do not replace the originals, but they help you describe the vehicle while a duplicate is processed.
Financed vehicles follow a different affidavit rule
Official sourceIf the CR is encumbered, the lost-CR affidavit is executed by the financing institution, not only by the borrower.
Older circulars listed extra documents
Confirm at officeA 2011 LTO memorandum on duplicate OR/CR listed HPG clearance and certified copies among other items. The Citizen’s Charter service page used for this v0.1 checklist does not print those as the current standard set. If your office still asks for HPG clearance, follow the office.
Where / How To Apply
LTO District and Extension Offices, generally 8:00 AM to 5:00 PM. Electronic confirmation from the originating office may be requested if the record is not in the transacting office.
A large part of this clock is records retrieval and possible electronic confirmation from another district office, not the cashier window alone.
