Passport for Minors
A minor must appear with a parent, legal guardian, or authorized adult companion. Prepare the PSA birth certificate or other proof of filiation, the companion’s ID, and an SPA when the companion is not the parent or guardian.
Philippine passport applications for applicants below 18, including parent, authorized companion, and legal-guardian accompaniment rules published in current DFA sources.
