Passport Name Change Supporting Documents

When your passport name must change, DFA generally requires original PSA-issued civil registry documents supporting the change—such as Certificate of Marriage, annotated Certificate of Live Birth, annotated Certificate of Marriage showing annulment/divorce/court instruction, and/or Certificate of Death of spouse. Women retaining maiden name typically do not need a marriage certificate for that choice. Reversion of maiden name has additional affidavit rules and is limited. Present these with the core renewal/new requirements. Passport name follows PSA records.

PSA-issued supporting documents commonly required when changing the name on a Philippine passport—used with renewal or related applications.
